Job Responsibilities:

General Description:

The medical records coordinator creates and maintains patient records according to established procedures and guidelines.

Specific Responsibilities of the Job:

  • Creates, organizes, and maintains all patient records (electronic and paper), including scanning, sorting, collating, filing and proper distribution of all information received regarding patients according to established procedures and guidelines. Verifies and assigns medical record numbers (utilizing the computerized patient registration system when available).Assembles, repairs, and reorganizes medical records as required.
  • Ensures that each patient’s information in the paper chart and electronic medical record (EMR) is accurate and that all required information is collected and entered into Clear Health based on agency and grant requirements.
  • Ensures that all medical records are maintained in secure locations and in compliance with HIPAA privacy regulations. Assures confidentiality of patient records by following established protocol and guidelines for medical records along with the proper procedures for medical information release and costs associated with the release of medical records if any.
  • Processes requests for copying medical records for transfer to other providers, healthcare facilities, etc. with appropriate documentation in the medical record.Follows up on requested records to ensure receipt of records in a timely fashion. Reviews all incoming medical records from other providers within determined time frame and summarizes patient records in a progress note and places it in the chart.
  • Performs continuous quality improvement in regards to medical records, i.e. assuring that any new system implementation is completed in a timely and accurate manner as well as staying abreast of changes in federal HIPAA laws regarding the storage of medical records.Identifies ways to improve work processes in the medical records area and to improve internal and external customer satisfaction.
  • Participates in employee events and meetings such as All Staff, Clinic Meetings, employee activities, workshops and other training.Takes initiative and responsibility for own continuing education and growth by attending in-service sessions, workshops, reading magazines or journals on patient care.Integrates knowledge gained into current work practices.Participates in clinical team meetings regarding client care, needs, procedural changes, etc.
  • Maintains a positive working relationship and effective communication with patients, providers, management and other employees to promote cooperation.
  • Cross-trains and maintains other position description and Medical Assisting skills.
  • Cross-trains all appropriate personnel to cover responsibilities when needed.
  • Other duties as assigned.

Job Qualifications:

Required Knowledge, Skills and Abilities:

Job Requirements:

  • Clinical medical records management preferred.
  • Proficiency in Word, Excel, Outlook and electronic medical records.
  • Excellent verbal and written communication.
  • Ability to handle multi-tasking
  • Detail oriented with a high level of accuracy.

    Education and Experience:

  • High school diploma or G. E. D.
  • Minimum 2 years experience in medical records management.
